(Ages 12 and under) $199 annually per individual includes:

  • 2 Regular Cleanings

  • Up to a 20 % courtesy for most dental procedures

Membership Limitations and Exclusions:

Any services which in the opinion of the attending dentist are neither necessary nor recommended for the patient’s health. Any service for which the patient is referred out, i.e. Oral Surgery, Endodontics, Periodontics, etc. Services for injuries or conditions that are covered under worker’s compensation or employer’s liability laws. Not valid for patients with insurances that are accepted at the practice. Not valid for Invisalign clear aligners, Zoom in-office whitening treatment or products available for purchase in the practice, i.e. toothbrushes, take home whitening kits, etc. Plan covers Full mouth or Panoramic X-rays once every 3-5 years. Elevation Dental Studio reserves sole discretion to refuse treatment to any plan member or dismiss any plan member as a patient.
